Office hours are Monday through Friday.
The telephone is answered each day from 9AM to 4PM (except for
Fridays, when the phone is answered until 2PM). The office closes for
lunch daily from noon to 1PM.
To the first office visit,
please bring your driver's license, current insurance card, and
relevant medical records from previous evaluations, including growth
records, lab reports, radiology reports, and biopsy reports. For
infants and toddlers, please bring the most recent dirty diaper so that
stool can be checked for blood that is not visible.
All patients under 18 years of age must be accompanied by a parent or legal guardian for each visit. |
